As your nurse advocate, I visit the patient in the hospital (when allowed) to speak directly with the discharge team. I advocate for appropriate follow-up care, home support, and resources before discharge plans are finalized.
Hospital discharge paperwork is often rushed and confusing. I meet with the patient and family to explain the instructions clearly—what medications to take, symptoms to watch for, appointments to schedule, and who to call with concerns.
After discharge, I provide a home visit to ensure the patient has what they need (equipment, meds, support), is following the care plan, and has a safe environment to recover—an essential step in preventing avoidable readmissions.
I make sure all the patient’s doctors—including specialists and primary care—are informed of the hospitalization and updated with new test results, medications, and follow-up needs. This prevents gaps in care that can lead to complications.
Patients are often prescribed new medications after a hospital stay. I reconcile all meds (old and new), check for duplications or harmful interactions, and help the patient understand what to take and when.
I maintain regular contact after discharge to monitor progress, ensure follow-up appointments are scheduled and kept, and address any warning signs early—reducing the chance of preventable setbacks.

Do patient advocates need a license? No, there is no formal licensing required, however, there is a certification that demonstrates competency in the field.
What is the difference between an independent patient advocate and a hospital/insurance advocate? Unlike hospital advocates, an independent advocate is private pay, so my alliance is with you. An advocate supplied by the hospital or insurance company will ultimately have their employer's interest at heart, first and foremost.
